Agreement to Terms

These Terms of Service ("Terms") govern your access to and use of the website located at https://www.protea-therapy.com (the "Site"), operated by Protea Physical Therapy & Wellness ("Company," "we," "us," or "our"), located at 130 S Chaparral Ct #140, Anaheim, CA 92808.

Description of Services

Protea Physical Therapy & Wellness provides pelvic floor physical therapy and related wellness services. The Site is intended to provide general information about our services, allow users to request appointments and discovery calls, and connect prospective patients with our clinical team.

Content on this Site — including blog posts, condition descriptions, and educational material — is for informational purposes only and does not constitute professional medical advice, diagnosis, or treatment.

No Provider-Patient Relationship

Submitting a contact form, scheduling a discovery call, or otherwise communicating with us through the Site does not create a provider-patient relationship. A provider-patient relationship is established only after you have completed our intake process, signed the applicable consent forms, and been formally accepted as a patient of the practice.

Please do not send detailed medical information, symptom histories, or other sensitive health information through the Site's general contact channels. Use the intake forms and secure communication methods we provide once you have been onboarded as a patient.

Appointments and Payment

Appointment requests submitted through the Site are requests only and are not confirmed until we contact you to schedule. We are a cash-based, out-of-network practice. We do not bill insurance directly. Superbills may be provided upon request for you to submit to your insurer, but we make no representation or guarantee regarding whether your insurer will reimburse any portion of your care. Specific fees, cancellation policies, and payment terms are provided separately at the time of scheduling and are governed by the agreements you sign as a patient, not by these Terms.

SMS / Text Message Program

By opting into our SMS program, you agree to receive text messages from Protea Physical Therapy & Wellness. The types of messages you may receive include:

• Appointment reminders – confirmations, reminders, and follow-ups related to your scheduled visits

• Alerts & notifications – important updates about your care, schedule changes, or practice announcements

• Promotional messages – occasional offers, wellness tips, or information about our services
